POROSIS-CARE (the "Service") is a web service providing general medical information about osteoporosis and helping you keep a health record. The Service does not provide medical care, diagnosis, or treatment.
You are responsible for the accuracy of the information you enter into the Service, such as bone density results and medication records. Anything displayed on the basis of that information is for reference only; if you intend to use it in any medical decision, confirm it with your own doctor.
The tools on this Service — the bone density and blood test readers, the list of questions for your doctor, and the appointment preparation sheet — are there to help you check what your figures mean and to print something you can take to an appointment. No registration is required, and there is no notification or reminder feature. For the schedule of any test or treatment, always follow your doctor’s instructions.
The information on this Service is not a substitute for a doctor’s diagnosis, treatment, or prescribing instructions. Always consult your own doctor before changing a treatment plan, reducing or stopping a medication, or starting a new treatment. Please do not use this Service as a reason to skip seeing a doctor.
This Service does not store what you enter. Calculations happen inside your browser alone, and what you entered is lost the moment you close or reload the page. Print what you need and take it with you. We recommend keeping your own records in a medication notebook or on paper.
To the extent permitted by law, the operator is not liable for damages arising from use of this Service. This does not apply where the damage results from the operator’s intent or gross negligence.

Each provision of these terms applies only to the extent that it does not conflict with the Consumer Contract Act or other mandatory provisions of law. If any provision is held invalid, the remaining provisions remain in effect.
These terms are governed by and construed in accordance with the laws of Japan. The Yokohama District Court shall have exclusive jurisdiction as the court of first instance over any dispute relating to this Service.
